CF-Related Diabetes and Fasting
Question
Assalamu alaykum,
I would really appreciate guidance on the following matter according to the Hanafi madhhab.
I have read that individuals with chronic illnesses may be exempt from fasting, and that fasting could even be sinful. However, I have also come across opinions stating that people with diabetes may still be required to fast and make up missed fasts. This has left me uncertain about my own situation.
I have cystic fibrosis and CF-related diabetes, both of which are chronic conditions. To manage my health, I take at least 9 different medications daily including multiple insulin injections. While I can choose when to take my medications, there are certain times of day that are medically optimal. Although my diabetes is currently well controlled, I am unsure whether fasting would be harmful in the long term.
I also came across the following resource discussing the medical risks of fasting with cystic fibrosis, which suggests that one may be exempt but should consult an imam:
https://www.cysticfibrosis.org.uk/about-us/resources-for-cf-professionals/supporting-clinicians/resources-for-clinicians/nutrition-leaflets/fasting-during-ramadan
My main concern is that while fasting may not be immediately life-threatening, it could potentially cause long-term harm. I am also underweight and struggle to gain weight due to cystic fibrosis, and I worry that fasting could worsen this. At the same time, I am concerned about whether I am sinful for not properly
exploring this matter and trying to take the easy way out by avoiding fasting without sufficient justification. My doctor is not Muslim, so I am unsure how to weigh their medical advice in this context.
With all this said, am I considered exempt from fasting and if not, how do I make up for 15 Ramadans of missed fasts without causing harm?
JazakAllah khair for your time and guidance.
Answer
As salām ʿalaikum wa raḥmatullāhī wa barakātuhu
In the Name of Allāh, the Most Gracious, the Most Merciful.
Fasting in Ramaḍān is a Divine and merciful obligation on every person who is able to do so. The principal ruling is, that if, a competent Muslim doctor advises one not to fast or if one genuinely fears, due to previous experience, that one will become sick by fasting or one’s sickness will be aggravated by fasting, then such a person is excused from fasting. However, such a person will have to keep Qaḍā fast for each missed fast in the future when one is able to do so.
Brother, in the case of diabetes specifically, when we consider whether the disease will be “aggravated” we look at the possibility of fasting increasing the risk of the following outcomes:
1- Hypoglycaemia. Low blood sugar <3.9mmol/l. This is a medical emergency and necessitates breaking the fast. It is often accompanied by tremors, sweating, light-headedness, confusion and coma.
2-Severe Hyperglycaemia. High blood sugar >16.6mmol/l. This is an indication to break the fast when accompanied by symptoms such as extreme thirst, fatigue, confusion etc
3- Acute illness. Emergencies of high blood sugar such as DKA or HHS. Other acute illnesses such as dehydration, heart attack, stroke etc
When assessing the risk of the above outcomes occurring doctors consider various factors, including but not limited to:
- The type of diabetes
- Duration of Diabetes
- Number and types of treatment the patient is on
- Diabetic control
- Presence, recency and frequency of low blood sugar episodes
- Glucose monitoring by patient
- Presence and severity of complication of diabetes such as recent hyperglycaemic emergencies, kidney damage, nerve damage etc
Due to the presence and severity of these factors varying between patients the advice to fast or not fast varies from patient to patient.
The doctor should assess these factors, after discussion with the patient, and assign them to be in one of the following categories.
1- Low risk. The patient must fast
2- Moderate risk. The patient ought to fast with close monitoring. If there is a need, the patient may break the fast.
3- High risk. They should not fast.
Cystic fibrosis is associated with malnourishment which may increase the risk of hypoglycaemia in the patient with CF who fasts. Furthermore, dehydration caused by fasting may lead to thickening of secretions in the lungs and intestines, causing difficulty with breathing and increasing the risk of constipation and intestinal obstruction. Core pulmonale (right heart failure due to lung disease) also frequently occurs in CF and may be aggravated by the fluid changes caused by fasting.
The complexity of your case necessitates an in-depth review of the risks by a Muslim endocrinologist to provide an individualised answer. If the conclusion is that fasting is likely to aggravate one or both conditions you should not fast and keep qadhā at a later point. If there is no hope of being able to fast in the future, one may pay the fidyah for the missed fasts.
And Allāh Taʿālā knows best.
